DOES MY INSURANCE COVER YOUR THERAPY?
INSURANCE

Jack is an “In Network Provider” for the following insurances and is continuously adding providers, so be sure to ask Jack if your insurance is in network.”

Simply send a picture of your insurance card before your first visit and Jack May will contact the insurance.

WHAT IS A DEDUCTIBLE?

A deductible is the amount of money you must spend on all medical services before your coverage begins. Once the deductible is met, you will have a co-pay. Jack will file claims and get paid by your insurance company. With in-network providers, you have a set co-pay. With out of network providers, your co-pay is based on the percentage your plan pays for therapy. Jack will check your deductible and rates of reimbursement to let you know what your co-pay will be.

These are a few of the well known insurance carriers, but there are many more.

OUT OF NETWORK

Insurance companies pay a portion of the fee (depending on your plan, typically ranges from 50-80%). Be sure to call and check your specific benefit plan to find out about your coverage for therapy. Simply send a picture of your insurance card before your first visit and Jack May will contact the insurance.

WHAT IF I DON'T HAVE INSURANCE?

A therapy session is typically 50-55 minutes long. All therapy sessions that are paid out of pocket, not through insurance, are $250 per session. If you have extenuating circumstances, please contact me to request a sliding scale/income driven discount.

Insurance Disclaimer

Capricorn Counseling Center is proud to accept and bill in-network insurance on your behalf to make services as convenient, accessible, and affordable to you. It is important that you update Capricorn Counseling Center with all of your insurance policy information to ensure accurate insurance billing. When you schedule an appointment, our team will do our best to verify your insurance benefits and quote your expected financial responsibility, known as cost-share, prior to your first appointment.

The information that we quote you is based on information that we obtain directly from your insurance carrier. A quote of benefits does not guarantee coverage and is subject to change based on your insurance policy’s processing of claims. Because information provided to us is not always 100% accurate, we highly recommend that you discuss your insurance benefits with your insurance company to fully understand the insurance policy benefits.

We are participating & are in-network, with most insurance plans offered above.
