Simply send a picture of your insurance card before your first visit and Jack May will contact the insurance.
A deductible is the amount of money you must spend on all medical services before your coverage begins. Once the deductible is met, you will have a co-pay. Jack will file claims and get paid by your insurance company. With in-network providers, you have a set co-pay. With out of network providers, your co-pay is based on the percentage your plan pays for therapy. Jack will check your deductible and rates of reimbursement to let you know what your co-pay will be.

Insurance companies pay a portion of the fee (depending on your plan, typically ranges from 50-80%). Be sure to call and check your specific benefit plan to find out about your coverage for therapy. Simply send a picture of your insurance card before your first visit and Jack May will contact the insurance.
A therapy session is typically 50-55 minutes long. All therapy sessions that are paid out of pocket, not through insurance, are $250 per session. If you have extenuating circumstances, please contact me to request a sliding scale/income driven discount.

When you schedule an appointment, our team will do our best to verify your insurance benefits and quote your expected financial responsibility, known as cost-share, prior to your first appointment.
